Fix README.md links and some formatting updates (#273)

* Some `README.md` whitespace and tweaks

* Fix link and title to GitHub documentation page on `README.md`

* Update link to `actions/toolkit` documentation

* De-quote some YAML blocks/examples in `README.md`
